Downloadable Forms (PDF)  The forms below can be filled out on your computer. For those who have a digital signature in place, you can sign it online and e-mail it to cms@drsusantaylor.com. For those who do not have a digital singnature in place, you can e-mail the form and print out the signature page, sign it and either scan it via e-mail or put it in an envelope and mail it to 99 Grimms Road, Honesdale, Pa  18431

Instructions:
Once you have downloaded and completed the form, use the menu to save the file so the completed information is saved. Once the form is saved, compose a new email message, then attach the completed form to the message and send it as an attachment in an email.

Note: It is recommended to right-click on the document links and save the PDF documents to your computer to be able to save your changes as you complete the form.
